  • What is the purpose of this form?
    • This form is used by off-site permit holders to notify the ATCC of their intent to participate in events such as farmer’s markets or non-profit festivals.
  • Who is required to submit this form?
    • Off-site permit holders (e.g., wineries, breweries, and distilleries) must submit this form.
  • What is the deadline for submission?
    • The form must be submitted by the 20th day of the month preceding the off-site event.
  • What additional documentation is required?
    • Retail off-site permit holders must attach a copy of their invitation to attend the event, and distilleries must have at least one certified alcohol awareness individual present.
