  • What is the purpose of this form?
    • This form is used by holders of public transportation, public storage & transportation, or individual transportation permits to order additional Vehicle Identification Cards (VIDs).
  • Who is eligible to request additional VIDs?
    • Only individuals or entities with existing public transportation, public storage & transportation, or individual transportation permits can apply for additional VIDs.
  • What is the fee for additional VIDs?
    • The fee is $10.00 per identification card.
  • Where do I send this form?
    • Mail the completed form along with the required fee to the Maryland Alcohol, Tobacco, and Cannabis Commission, Regulatory & Research Section at 1215 E Fort Avenue, Suite 300, Baltimore, MD 21230.
  • How long is the application process from start to finish?
    • An applicant can complete filling out the application form in 3-5 minutes.
    • Upon receipt of the completed application and required payment, the verification and determination on issuing said application may take 10-30 business days.
