  • What is the purpose of the Direct Delivery Form?
    • This form is required for every direct delivery of alcoholic beverages from a manufacturer to a consumer in Maryland.
  • What details must be included?
    • The form requires the purchaser’s name, delivery address, recipient’s identification, and details of the alcoholic beverages delivered.
  • What is the retention period for these forms?
    • Licensees must retain completed forms for at least 3 years.
  • What is the certification process for delivery?
    • Both the recipient and the deliverer must sign the form, confirming the delivery and receipt of alcoholic beverages.
  • What is the permit fee? 
    • The permit fee is $200.
  • How long is the application process from start to finish?
    • An applicant can complete filling out the application form in 3-5 minutes.
    • Upon receipt of the completed application and required payment, the verification and determination on issuing said application may take 10-30 business days.
